- In What Lies Over the Cuckoo's Nest? there is a missing insane person in one of the chambers, who is substituted by Bruce Elm. According to a fan theory, Shivers used to be held in the hospital but died and Dr. Krauss had to find a replacement to power the device keeping the Darkling hostage.
- The character's design is inspired by the 1990 movie, Jacob's Ladder.
